Sam Chapman, Founder and Lead Investigator
Sam Chapman has spent more than a decade in the places where risk shows up in person. He started in campus public safety and spent seven years in hospital security supervision at a major Seattle academic medical center. There he handled the incidents most organizations only plan for: threats against staff, volatile patients and visitors, and the documentation that follows.
He went on to investigate abuse, neglect, and exploitation of vulnerable adults in state-agency investigations. The work depends on careful interviewing, clean evidence, and findings that hold up on review.
That combination is the foundation of Telos. Sam can interview a reluctant witness in the morning and brief a general counsel in the afternoon, and he writes reports that both audiences can use.

Trent Bazis, Licensed Private Investigator
Trent Bazis is an experienced investigator based in Seattle, Washington, with a background in legal compliance and public sector investigations. Bringing a rigorous, detail-oriented approach to complex casework involving regulatory compliance and legal accountability, Trent has built a career grounded in the principles of due process, thorough evidence gathering, and impartial fact-finding. A graduate of the University of Nebraska-Lincoln, he is committed to delivering objective, professional investigative work in service of both clients and the public interest.

Why “Telos”
Telos is the Greek word for purpose: the end a thing is meant to serve. We chose it because investigations go wrong when they lose sight of theirs. Hours get billed, reports get long, and the client still cannot answer the question they came in with.
Telos Risk Intelligence was founded in 2025 to work the other way. We start with the decision you have to make, whether that is a settlement, a claim determination, a termination, or a protection plan, and we build the work to serve it.
